Physical Damage Coverage for Boats & Waverunners

RELEASEE has offered physical damage insurance coverage to the EQUIPMENT only. There is an additional fee of $35.00, per boat or wave runner with a $500 deductible.  I understand that if I waive the physical damage insurance coverage I will be responsible for any and all damages to the equipment.

I AGREE TO PAY FOR ANY AND ALL DAMAGES TO THE EQUIPMENT. I THE LESSEE/USER GIVE MY CONSENT TO CHARGE THE CREDIT CARD USED FOR THE RESERVATION OR MY CREDIT CARD FOR ANY AND ALL DAMAGES WHICH I MAY INCUR, OR  I AGREE TO PAY THE $500 DEDUCTIBLE WITH THE PURCHASE OF THE OPTIONAL PHYSICAL DAMAGE INSURANCE COVERAGE AND GIVE MY CONSENT TO CHARGE MY CREDIT CARD (COVERAGE IS  FOR THE LEASED EQUIPMENT ONLY )
